This form is used to schedule a DNA
collection at our facility in Euless, Texas.
Collection costs are $20.00 per
person for buccal swab including returning samples by priority mail.
Regular hours:
Monday-Thursday 8:30am to 6:00pm
Friday 8:30am to 5:00pm
Saturday
and Sunday By appointment only
