Use this form to request a refund if your test must be
cancelled.

Self collection tests (home kits) will be
completely refunded less the shipping fee. |

Legal tests shall be completely refunded less
any collection fees plus shipping costs plus $150.00 for case
management (preparation of the legal chain of custody documentation,
consultation, appointment scheduling, shipping) and $50 for
each additional collection location. |
|
There are no special sample (chewed gum,
cigarette butts, bone, hair, etc) refunds once a test has
begun. |
